Documentation & Billing

This section provides the forms and guidance needed for monthly documentation and billing. All forms should be downloaded, completed, and submitted via email according to your contract requirements.

Documentation is due by the 15th of the following month. Invoices should be submitted within 15 days following the end of each month.

Reporting

Reporting helps demonstrate the impact of your services and informs future funding decisions. These reports are essential to sharing outcomes, identifying challenges, and supporting continued collaboration.

Resources:

Event Reporting Form
Submit within 24 hours of any reportable incident.

Quarterly Reporting Portal
Complete required reports through the Greater Kansas City Community Foundation.

Navigating Quarterly Reports
Quarterly reports are due:

  • April 15

  • July 15

  • October 15

  • January 15

Reports should provide a clear picture of services delivered, outcomes achieved, and overall program impact.
