Documentation & Billing
This section provides the forms and guidance needed for monthly documentation and billing. All forms should be downloaded, completed, and submitted via email according to your contract requirements. Eitas is open to receiving documentation in other formats as long as it includes the information found on the forms below, please ask your Agency Relations Specialist if you have any questions.
Documentation is due by the 15th of the following month. Invoices should be submitted within 15 days following the end of each month.
